What to check
- Re-enumerate addresses/phones/devices from the current provider.
- Correlate USB, PnP, service-provider, and TAPI reinitialization events with the disappearance.
- Check whether the application is holding an identifier from before a device removal or provider restart.
Handling
Refresh device enumeration and obtain a new object/identifier. If the device is physically required, restore it before creating a new call or phone operation.
