The storage topology changed underneath the caller
This status is a stale-topology guard. It means the caller issued an operation using a topology identity that no longer matches the storage stack, often after device arrival/removal, multipath change, failover, or virtualization update.
Capture the topology identifier expected by the caller and the identifier reported by the current storage path. Hardware replacement, path changes, or stale cached topology can explain the mismatch.
What to inspect
