Capture the exact open conditions
- Check whether the file still exists and whether a rename or deletion raced the scan.
- Inspect sharing violations, ACLs, encryption, offline state, reparse behavior, and antivirus or backup locks.
- Determine whether the module requested read content, write content, or metadata-only access.
Do not assume administrative access in an interactive shell matches the module account used by the pipeline. Modules that need content receive a stream only when their definition declares that requirement, and write access has additional storage-module constraints. Reproduce with the same account and access mode, then decide whether the failure is transient or the file should be excluded.
