What this result proves
This code proves that the requested path could not be resolved to a file. It does not, by itself, prove a codec, permissions, network, device, or media problem outside that boundary. Preserve the first lower-level result when one exists, because later Player or Encoder errors can hide the original cause.

Record the exact expanded path or url-to-path mapping, current working/base directory, and service identity; verify that the same identity resolves that exact path. Keep the media object, endpoint, profile, or device identity fixed while changing only that rejected precondition. A successful retest should pass this exact boundary; a different HRESULT from a later stage is new evidence, not recurrence of NS_E_FILE_NOT_FOUND.
