NS_E_CD_REFRESH (0xC00D0FB1) is a concurrency/state result around drive table-of-contents refresh. It does not mean the disc contents are invalid.
Find the overlapping request
Record thread/callback, refresh start/completion, drive identity, and UI or background operation that issues the second refresh. Media-change notifications can trigger another query while one is still active.
Correct sequencing
Allow the current refresh to finish, coalesce duplicate requests, and then read the updated CD state. Persistent read errors after serialization belong to the CD reader path, not refresh locking.
