MF_E_WMDRMOTA_ACTION_ALREADY_SET points to OTA lifecycle or configuration ordering. An output trust authority represents a single action. Once the WMDRM OTA has been configured for that action, a second action-assignment attempt is rejected.

Find duplicate initialization rather than retrying policy

Log OTA creation, action assignment, and the first SetPolicy call with an object/session identifier.
Check error recovery that re-enters configuration on an already initialized OTA.
Verify that one OTA object is not being reused for different play, copy, or transcode requests.

The error is distinct from an unsupported action reported by an ITA and from MF_E_WMDRMOTA_ACTION_MISMATCH. Here the immediate problem is that action state has already been committed. Creating a new policy object or resubmitting the same call does not reset the OTA.
