STATUS_FLT_DUPLICATE_ENTRY
STATUS_FLT_DUPLICATE_ENTRY is normally discovered during filter registration. The operation-registration table contains more than one handler definition for an operation that Filter Manager expects to have one registration entry.
Review the generated or hand-written FLT_OPERATION_REGISTRATION table, including conditional compilation. The intended major function should appear once with its pre-operation and optional post-operation callbacks, followed by the required table terminator. Do not rely on table order to choose between duplicates; the configuration is invalid before I/O dispatch begins.

- Two feature blocks registering the same major function.
- A pre-operation and post-operation pair mistakenly written as separate entries.
- Code generation that appends a second table fragment for an already enabled feature.
