STATUS_VOLMGR_ALL_DISKS_FAILED
The failure is at pack scope, not one volume extent
STATUS_VOLMGR_ALL_DISKS_FAILED is the pack-wide failure case. A dynamic-disk pack groups disks that participate in one LDM configuration; Microsoft documents that each dynamic disk stores configuration information about the other dynamic disks in the computer.
When every disk belonging to the pack is failed or unavailable, Volume Manager cannot treat the problem as a single stale member or one degraded plex. The status therefore points first to storage visibility and pack recovery, not to a file-system repair on one drive letter.
The code does not prove that all media failed independently. A shared controller, enclosure, SAN path, power event, or virtual storage backend can make several pack disks disappear together, so correlate the timestamp with lower-level storage events before changing disk metadata.
What to inspect
- Record which dynamic disks are Offline, Missing, or Foreign and whether their identifiers still match the expected pack.
- Check controller, multipath, enclosure, and hypervisor storage events for one failure domain affecting every member.
- Avoid initializing, cleaning, or converting the disks while pack membership and recoverability are still being established.
