The whole pack object failed validation
This status is broader than an invalid pack name or identifier: the Volume Manager pack record itself is not acceptable for the requested action. Treat it as configuration metadata validation, not proof of a bad sector or a single offline volume.
Diagnose at pack scope
Record pack identity, state, flags, member disks, quorum status, and the exact request. Compare the configuration view on each available member before attempting an import or modification. A complete inventory is needed because the pack object represents a group of dynamic disks, not one disk alone.
