What does macOS kernel return 0xDC004003 (kOSMetaClassNoInit) mean?

kOSMetaClassNoInit

kOSMetaClassNoInit represents a broken OSMetaClass registration sequence: XNU logs that preModLoad() was not called before a class constructor attempted registration. Metaclass constructors are expected to run between the preparation and completion phases that establish the staging context for a kext's classes.

The current XNU source marks this status as “never returned” and logs the failure at the call site. That makes it mainly useful when reading legacy kernel logs or code derived from older XNU trees; it is not a normal application-visible device result.
