This is the intra-coded counterpart of the MCBPC failure. The bit pattern does not map to a valid intra macroblock type and chroma coded-block pattern.
Check whether the decoder entered the intra path correctly. A wrong picture header or corrupted macroblock mode can select the wrong table and make valid bits look invalid.
