STATUS_FVE_FAILED_AUTHENTICATION

This code means an authentication path was actually attempted and did not succeed. It is more specific than STATUS_FVE_LOCKED_VOLUME, which only says that an encrypted volume has not been unlocked.

Match the recovery-key identifier shown by the target with the record from the approved recovery store before entering a 48-digit recovery password or using a recovery key. A key that belongs to a different volume can be syntactically valid yet unable to unlock this one.

Do not clear a TPM or remove protectors to bypass a failed recovery attempt. Preserve the recovery record and investigate whether the wrong volume, wrong protector, or an unexpected boot-state change triggered the request.
