Evidence to preserve
Capture the disk UID, server, block or offset, requested byte count, bytes actually returned, and the earliest filesystem or device error for that read.
How to interpret it
A successful read from another member does not clear this result. The useful question is whether the same disk and block remain unreadable through the same Content Server path.
Recovery and verification
Repair or replace only the failing storage path identified by the lower-level error. Retest the same read before starting a broader rebuild or changing the server layout.
