This differs from STATUS_GRAPHICS_OPM_INVALID_SRM. Invalid SRM means an attempted update was unacceptable; never set means there is no successful update whose version can be queried. Treat the answer as unavailable, not as a zero version.

- Record protected-output creation time, SRM set attempts, and output recreation events.
- Do not use a version result from a previous output object after a topology or session transition.
