ERROR_GRAPHICS_MONITOR_COULD_NOT_BE_ASSOCIATED_WITH_ADAPTER means a newly detected display could not be matched to the adapter target that should own its descriptors, modes, and connection state. The failure can follow unstable child IDs, duplicate target identities, a hot-plug race, or monitor data arriving after the target was removed.
Correlate the child-status notification, target ID, connector identity, and EDID read within one connection generation. Re-reading EDID alone will not fix a target mapping error. On switchable graphics, docks, or multi-stream hubs, also verify which adapter enumerated the child and whether ownership migrated while asynchronous monitor discovery was still running.
