A JPEG 2000 code-block is decoded through a sequence of bit-plane coding passes. The pass number must refer to an existing pass for that block; a value outside the allowed range means the block metadata and pass list disagree.
Look at packet header parsing first. Incorrect included-block, zero-bit-plane or pass-count information can make the decoder request a pass that was never present.

- The number of coding passes declared for the code-block.
- Packet header fields that add passes to the block.
- Layer progression and whether tile-parts were assembled in order.
